RETAINERS FOR LIFE PLUS- END OF TREATMENT
Your partnership gives patients two ways to protect their smiles:
1. The Smile Protection Plan (Membership)
2. Pay As You Go
The Smile Protection Plan: After enrolling (one time fee per arch), patients gain access to wholesale-priced retainers that can be conveniently shipped directly to their homes*.
The Smile Protection Plan includes one retainer per enrolled arch. With End of Treatment customization, this included retainer serves as the patient’s debond retainer--as long as they enroll on or before the Debond -1 Appointment.
If the patient enrolls after the debond appointment, the included retainer instead becomes a backup retainer. Patients should NOT be informed about the included retainer prior to the Debond Appointment.
The Pay As You Go option is perfect for patients who prefer not to join the membership. It allows them to easily order replacement retainers for home delivery whenever needed*.

Use this form to enroll a NEW PATIENT who needs a payment plan longer than what is offered online. This form will enroll a patient and set up a payment plan but NOT automatically process a retainer order. If a retainer order is needed send the scan to the RFL Lab with RX notes included.
